How to Enroll

There are two options when it comes to enrolling with Ombudsman. Each one has been crafted to meet the individual needs of each student. Review the list below to see what best fits your situation.
Enrolling Through Your District
School districts refer students to Ombudsman using specific criteria created in partnership with the Ombudsman implementation team. Intake appointments are scheduled for students and parents/guardians so they can learn what the program can do for them and all the requirements involved.

Private Tuition
The 2nd option for many students is to enroll as a private-pay student. In these cases, students and parents choose Ombudsman on their own and incur all costs associated with the program.

AZ Charter Locations

Charter Application Form. If you are filling out the above application, you may either print it out, fill it out by hand and fax it to 602-840-1402 OR you can fill it out electronically. To fill out electronically, under the file menu, choose ‘save as’ and save it onto your computer, then click on each blank and type in your information. Attach the finished form (one page) to an email and send to jmattos@esa-education.com. If you have difficulty with the form or the email, please call 602-840-2997.
